Blank 100-Question Bubble Sheet, No Name Block (A–D, Letter)
This hundred-question grid drops the name block so the rows can spread out: two columns of 50, A to D, bubbles 3.6 mm wide. Nothing on the page identifies a student. It is one US Letter page, a free PDF, and a sensible choice for anonymous benchmark papers.
| Questions | 100 |
|---|---|
| Choices | A–D (4 per question) |
| Columns | 2 × 50 rows |
| Paper | US Letter (8.5" × 11") |
| Recommended print scale | 100% / Actual size (turn off “Fit to page”) |
| Pages | 1 |
| Bubble width | 3.6 mm (0.14 in) |
| Row spacing | 4.7 mm (0.19 in) |
| Page margin | 10.6 mm (0.42 in) |
| Corner alignment marks | Included: four filled squares, 4.9 mm (0.19 in) from each edge |
| Header fields | No header block |
One hundred questions, two columns, one page, and nothing at the top but bubbles. Removing the header gives the grid a little under an inch more height, so the rows breathe a little more and the bubbles print slightly larger than on the headed version.
This is the sheet for anonymous hundred-question assessments, for practice sets you will reuse, and for anyone who wants the largest possible bubbles at this length without moving to four columns.
Who this sheet is for
- Anonymous full-length assessments
- Reusable practice sets
- Largest bubbles available at 100 questions in two columns
- Benchmark testing with names recorded separately
When to choose this sheet, and when not to
Choose the header-free hundred when you want the largest bubbles that two columns can give at this length and you do not need names on the sheet, either because the assessment is anonymous or because you record identity another way.
The gain is real but modest: compared with the headed 100-question sheet, bubbles grow a little and rows spread slightly. If bubble size is the main problem, the four-column version gives a bigger step up.
Without a header, you will want some other way to know whose sheet is whose, if you need to know at all. Numbering the sheets before handing them out is one simple option, and a rubber stamp in the top margin is a workable alternative. Whatever you choose, decide it before the test, not while you are collecting a hundred sheets.
An example
A district runs a hundred-item benchmark in many schools. Sheets arrive pre-numbered and carry no names, and results are returned to schools by sheet number.
How to print this at 100% scale
- Open the PDF and choose File → Print.
- Set the scale to Actual size or 100%. Turn off “Fit to page”, “Shrink to fit” and any borderless option.
- Set the paper size to US Letter (8.5" × 11").
- Print one copy and check the four corner squares: each should sit about 5 mm (3/16 inch) in from the paper edge.
Why the fuss? A page scaled by even two percent moves every bubble slightly, and a camera grader measures where a bubble should be. Print at true size and the grid is where the grader expects it; print scaled and it may not be.
Printing note. With no header, the grid begins just below the top corner marks — a useful indicator that your printer is not trimming the page edge.
How to use it
- Print one copy per student at 100% scale, single-sided.
- Have students fill in the header block first — name, date and class, and the Student ID grid if this sheet has one.
- For each question, fill one bubble completely. A half-filled bubble is a common cause of a mark being missed, and an X or a check mark can be another.
- Erase cleanly and completely. Smudged erasures can read as filled bubbles to a camera.
- Grade against an answer key — see the guide on making a test key overlay.

Frequently asked questions
How much bigger are the bubbles without the header?
Somewhat larger: the header block on the headed version takes a little under an inch of height, and that space goes back into the grid here. The specs tables on this page and on the headed sheet give both sizes.
How do I match sheets to students?
Number the sheets before handing them out, or have students write their name in the margin at the top of the page. If your grading tool supports ID grids, choose the sheet with the Student ID grid instead.
What print settings are correct?
Actual size or 100%, with all fit-to-page options off. A one- or two-percent rescale is invisible to the eye but enough to move every bubble off the position a grader expects.
Is anything uploaded when I download?
No. The PDF is a static file and nothing you do with it is sent back. The privacy page lists exactly what is and is not collected.
Is there a version of this layout with a Student ID grid?
Not as a ready-made file. The generator can build a hundred questions with a header and a Student ID grid, at the cost of smaller bubbles because the grid takes height from the answer rows.
Can I add my own header with a pen?
Yes, in the margin above the grid, but keep clear of the corner squares and the first row so nothing is mistaken for a bubble.
