Pre-printed bubble forms are convenient until you need them in a hurry, in a quantity the supply closet does not have, or in a length your test does not match. A sheet you print yourself costs a few cents of paper and can be the right size on the day you need it. This page is a 100-question, five-choice sheet in four columns of 25 rows, made for exactly that.

It is an original, generic layout. It is not affiliated with Scantron Corporation and it is not a copy of any Scantron form. Sheets like this one are meant to be filled in with a pencil and graded by hand, with a cutout overlay key, or with a phone-camera app you have tested yourself. A school's scanning machine reads only the forms it was set up for, so if your school scans with one, keep using those forms.

Who this sheet is for

  • Teachers who want to stop ordering pre-printed forms for hand-graded tests
  • Unit and final exams of up to 100 questions
  • Classes that grade with a punched or cutout overlay key
  • A spare stack for the day the supply closet runs out

When to choose this sheet, and when not to

Choose this sheet when you want an ordinary-looking 100-question answer sheet without ordering forms. Four columns keep the bubbles large enough to fill quickly, and one PDF covers a whole class set.

For a shorter test, use the 50-question A–E sheet, which has two columns of twenty-five, or the 50-question four-choice sheet. Both print on the same US Letter paper. For any other length, the generator builds a sheet from 1 to 300 questions.

Be clear about what the sheet is for. It is made to be marked in pencil and graded by hand or with an overlay key you make in the generator. Before you rely on a camera or scanning app, print one sheet, fill it in and grade it to see how that app reads it. We have not tested any.

An example

A teacher with five sections prints a class set of this sheet for each unit exam, grades the stack with a cutout overlay made in the generator, and keeps a spare ten pages in the folder for makeup tests.

  1. Open the PDF and choose File → Print.
  2. Set the scale to Actual size or 100%. Turn off “Fit to page”, “Shrink to fit” and any borderless option.
  3. Set the paper size to US Letter (8.5" × 11").
  4. Print one copy and check the four corner squares: each should sit about 5 mm (3/16 inch) in from the paper edge.

Why the fuss? A page scaled by even two percent moves every bubble slightly, and a camera grader measures where a bubble should be. Print at true size and the grid is where the grader expects it; print scaled and it may not be.

Printing note. Five bubbles in each of four columns makes a dense page. Print at 100% and check that the fourth column sits fully inside the right margin before you copy a class set.

How to use it

  1. Print one copy per student at 100% scale, single-sided.
  2. Have students fill in the header block first — name, date and class, and the Student ID grid if this sheet has one.
  3. For each question, fill one bubble completely. A half-filled bubble is a common cause of a mark being missed, and an X or a check mark can be another.
  4. Erase cleanly and completely. Smudged erasures can read as filled bubbles to a camera.
  5. Grade against an answer key — see the guide on making a test key overlay.

Frequently asked questions

Is this a Scantron form?

No. It is an original layout that we drew ourselves. It is not a copy of, or affiliated with, Scantron Corporation or any of its forms.

Can I run these sheets through a school scanner?

Probably not. Scanning machines read only the forms they were set up for, and we have not tested these sheets on any machine or app. Ask whoever runs your scanner before you print a class set. Hand grading and an overlay key always work.

How do I grade 100-question sheets without a scanner?

Make a cutout overlay key in the generator: mark the answers, download the overlay, cut out the dashed circles and lay it over each sheet. Count the filled bubbles that show through, after checking for rows with two marks.

Is there a 50-question version?

Yes. The 50-question A–E sheet and the 50-question A–D sheet are both on this site, on US Letter in two columns. The generator can build other lengths as well.

Do I need special paper or a special printer?

No. Print the PDF at 100% on ordinary US Letter paper from any printer. A soft pencil and a clean eraser keep the marks clear.

This is an original, generic layout for classroom and practice use. It is not affiliated with, endorsed by or compatible with any commercial scanning brand or any official exam. Always test-print one copy before running a class set.
